New patients
We are accepting new patients. Here is everything that happens between booking your first appointment and sitting down.
The forms
After you book, a link arrives by text or email. The forms take about ten minutes and are completed on your phone, which means the appointment starts when it is booked rather than fifteen minutes later with a clipboard.
If you would rather fill them in on paper, say so and we will have them ready at the desk. Nobody is made to use their phone.
What to bring
- Your insurance details, if you have coverage.
- A list of any medications you take.
- The name of your previous dental office, if you have one.
- Anything you have been worried about and want looked at.
Records from another office
If you have been seen elsewhere, we can request your records and recent images so that nothing is repeated unnecessarily. That request needs your written consent, which is part of the new patient forms.
Transfers usually take a couple of weeks. Your first appointment does not wait on them, and if they arrive afterwards they are simply added to your file.
Getting into the building
The office is on the ground floor with level access from the parking area and no steps at the entrance. There is accessible parking directly outside, and the washroom is accessible.
If you need something specific to be comfortable, tell us when you book rather than on the day. Longer appointment times, a quieter part of the day, and support people in the room are all straightforward if we know ahead.
Nervous about dental visits
Say so when you book. It changes how much time is set aside and how the appointment is run, and it is an extremely common thing to tell us.
You can also ask to come and see the room before booking anything at all.
Your information
A short version. The full detail is on the privacy page.
Health information collected on the forms is used to provide care and to contact you about appointments. It is not sold, and it is not used for marketing.
Appointment reminders are sent only to the channel you chose, and you can stop them at any time by telling us once.
You can ask to see what is held about you, and ask for corrections.
